Information to bring to our initial consultation.
***INFORMATION REQUIRED AT TIME OF APPOINTMENT***

You need to bring 2 PHOTOCOPIES of the items below.  I do NOT want originals.

Complete the worksheet here, online or, print it out and bring it to the initial interview.

1. Billing Statements to include:
(a) complete address together with zip code
(b) account number
(c) how much is owed and what it is for every debt you have must be listed, even those you wish to pay

2. Your residential addresses for the last two (2) years.

3. A list of financial and bank accounts, closed for the last one (1) year, including the name and address of each bank together with account numbers.

4. A copy of your pay stub showing net and gross income for the last 60 days. Additionally the attorney will ask you for your gross earnings for the prior six (6) months, not including the month of your initial consultation. If you are self employed, then a month-to-month Profit and Loss statement will be required for the past six (6) months.

5. The payoff amount on any automobiles that you are financing (try to bring a statement showing the payoff figure) Copy of registration of vehicle, if encumbered. Copy of proof of insurance. Information as to mileage on vehicles.

6. If you acquired a motor vehicle within the past 120 days, a copy of the purchase and/or lease contract.

7. Copy of all judgments and lawsuits and liens pending against you. If your wages are being garnished, a copy of the wage garnishment order will expedite our office stopping this action.

8. Any and all information you have regarding student loans, tax obligations or co-signed obligations, if applicable.

9. Your last two (2) years Federal tax return

10. A payoff amount on any real property that you are financing.

11. A copy of any Trust instrument in which you have a beneficiary interest.

12. If you are married and filing a separate case the following:
(a) A copy of any prenuptial agreement;
(b) All of the above information i.e. paragraphs 1 through 10, concerning your spouse, whether or not your spouse intends to join the bankruptcy
